Recommending that Ivermectin be dropped from the clinical guidance, experts cited 13 systematic reviews of which “7/13 showed mortality benefit, 4/13 no mortality benefit, 2/13 inconclusive/unclear.”

Additionally, there was a high risk of bias in many of the studies, particularly with the ones showing mortality benefit, as the level of certainty is low in them.

https://www.idsociety.org/practice-guid ... anagement/

By now, many Americans know someone who's been fully vaccinated but still got a breakthrough infection of COVID-19. To be clear, the Centers for Disease Control and Prevention still says that these cases are uncommon and only a tiny percentage of those uncommon breakthrough infections require hospitalization. But back in May, the CDC stopped tracking breakthrough infections unless they were severe. And that was before the highly contagious delta variant drove a new surge of COVID-19 throughout the country. So is this still the best practice? Well, vaccine expert Saad Omer joins us now. He's also the director of the Yale Institute for Global Health.

CDC no longer tracking all ‘breakthrough’ COVID-19 cases
August 10, 2021 at 7:35 pm PDT
By Gary Horcher, KIRO 7 News
After attending an outdoor wedding in Snohomish County, Chuck Wotipka was informed several people had tested positive for COVID-19 in the days that followed. Wotipka was vaccinated, so he figured his odds of becoming sick were low.

And then the symptoms hit him. “I was pretty much bedridden with extreme fever, stuffy nose, sore throat, just like a very bad case of the flu,” he said. Soon, he discovered he is among a growing number of people with “breakthrough” COVID-19 cases (those fully vaccinated and still got sick).

He was even more unpleasantly surprised that the Centers for Disease Control and Prevention does not track breakthrough cases like his unless there is a hospitalization. “For the general public to understand that more people are getting sick is really irrelevant information,” he said. “But if I’m a vaccinated person, and I understand there are a significant number of people like me getting sick, I might take more precautions because I think we all become a little more complacent after becoming vaccinated.”

The Snohomish County’s health district told KIRO 7 News it does track breakthrough COVID-19 cases and collects details it doesn’t publish on the website.

It tries to track which specific COVID-19 vaccines show up more often in breakthrough cases. The data shows over a three-week period between July 1 and July 20, there were 208 people infected with COVID-19 who were at least partially vaccinated —which equals 17% of all positive COVID-19 cases.

Its data for June shows if someone in Snohomish County is unvaccinated, they are 10 times more likely to become infected or die of COVID-19.

The CDC indicated on its website that it no longer has the staffing or resources to gather and process all data regarding breakthrough cases. Thus, it will concentrate on severe cases, hospitalizations and deaths. Wotipka believes more data regarding specific vaccines being involved in breakthrough cases will be more than “fun facts.”

“More data, more information saves lives,” Wotipka said. “The more information we have, the better off we’ll all be.”

Beginning May 1, 2021, CDC transitioned from monitoring all reported COVID-19 vaccine breakthrough infections to investigating only those among patients who are hospitalized or die, thereby focusing on the cases of highest clinical and public health significance. CDC will continue to lead studies in multiple U.S. sites to evaluate vaccine effectiveness and collect information on all COVID-19 vaccine breakthrough infections regardless of clinical status.

COVID-19 Vaccine Breakthrough Case Investigation and Reporting
Updated Oct. 28, 2021

For local health departments, healthcare providers, and clinical laboratories

CDC encourages local health departments, healthcare providers, and clinical laboratories that identify a COVID-19 vaccine breakthrough case to:
Request the respiratory specimen be held for further testing.
Report the case to the state health department where the individual resides for further investigation and reporting to the national system.

COVID-19 vaccine breakthrough cases that result in hospitalization or death should be reported to the Vaccine Adverse Event Reporting System (VAERS)external icon.

For state health departments

If a possible vaccine breakthrough case is identified:
Request that the clinical or public health laboratory hold any residual respiratory specimens from the positive SARS-CoV-2 test.
Report the available case data to NNDSS, per normal procedures.

Review CDC’s screening questions to assess whether the case meets the COVID-19 vaccine breakthrough investigation criteria.
If the reported case meets those criteria, CDC encourages state health departments to:
Follow the steps for initiating a COVID-19 vaccine breakthrough case investigation.
Record the case in the COVID-19 vaccine breakthrough REDCap database.
Because CDC would like to characterize the SARS-CoV-2 lineages responsible for COVID-19 vaccine breakthrough cases, including variants, CDC requests state health departments to:
Report sequence results from a state public health laboratory, commercial reference laboratory, or academic laboratory by entering the PANGO lineage and GenBank or GISAID accession number into the COVID-19 vaccine breakthrough REDCap database.
If SARS-CoV-2 sequencing will not be performed locally and an acceptable clinical respiratory specimen is available, provide instructions for the testing laboratory to send the residual respiratory specimen to CDC.
For cases with a known RT-PCR cycle threshold (Ct) value, submit only specimens with Ct value ≤28 to CDC for sequencing. (Sequencing is not feasible with higher Ct values.)
If the Ct value is not known (e.g., positive by antigen test only or by a molecular test that does not provide a Ct value), submit the positive specimen may still be submitted to CDC for RT-PCR and possible sequencing.

At the end of April, out of the 95 million Americans who had been vaccinated, the CDC said 9,245 breakthrough cases were reported to the federal agency. Included in the data: 835 hospitalizations and 132 deaths.

May 1, the CDC announced it would no longer track every breakthrough COVID-19 infection.

The agency’s website said:

“The CDC transitioned from monitoring all reported vaccine breakthrough cases to focus on identifying and investigating only hospitalized or fatal cases due to any cause. This shift will help maximize the quality of the data collected on cases of greatest clinical and public health importance.”

Nope. No sé.

COVID-19 Vaccine Breakthrough Cases: Data from the States
Jul 30, 2021
While COVID-19 vaccines are highly effective at preventing severe disease, hospitalization, and death from COVID-19 and also reduce the likelihood of mild or asymptomatic infection, a small share of fully vaccinated individuals do become infected, and some become hospitalized or have died. These rare occurrences are known as “breakthrough cases” which are to be expected, and historically known to occur with other vaccines as none is 100% effective.

The Centers for Disease Control and Prevention (CDC) currently monitors hospitalizations and deaths, from any cause, among fully vaccinated individuals with COVID-19, but not breakthrough infections, which it stopped monitoring as of May 1. CDC presents this data in aggregate at the national level but not by state, and there is no single, public repository for data by state or data on breakthrough infections, since the CDC stopped monitoring them.

We therefore reviewed the websites and other official state sources for all 50 states and D.C. to see which are providing data on COVID-19 breakthrough cases, hospitalizations and deaths, how regularly, and what those data may tell us. We only used data from official state sources (we did not include data available only in news media reports, for example). Where a state did not provide comparable data on overall COVID-19 cases, hospitalizations, or deaths reported over the period in which it captured breakthrough events, we obtained data on cases and deaths from the Johns Hopkins University COVID-19 Dashboard and on hospitalizations from the U.S. Department of Health & Human Services for the appropriate period (see methods for more detail).
Breakthrough cases – infections among vaccinated people – are happening. These are normal and expected because the vaccines, though all powerfully protective against Covid-19, are not 100% effective. But how many people, and which populations, are having breakthrough infections? And what are the chances they will develop long Covid, the cluster of debilitating symptoms that can last for weeks or even months?

It’s difficult to answer these questions because there’s a dearth of rigorous data on breakthrough cases in the United States. The US Centers for Disease Control and Prevention (CDC) tracks only breakthrough cases that lead to hospitalization and death, which it does by gathering data from state health departments. Only 25 states report some data on breakthroughs, and only 15 of those states update it regularly, according to a recent Kaiser Family Foundation analysis of state data. Encouragingly, this data suggests that breakthrough cases among the fully vaccinated are extremely rare – well below 1% in states collecting this information. (Note that undercounts are expected, since people with breakthrough infections may not know they are sick or bother to get tested.)

This is the kind of information that the public needs in order to understand the risks of daily life during the pandemic, and that scientists need to assess the shifting epidemiology of the virus. We should be able to rely on the CDC to collect and share this data. Instead, we are relying on nonprofits, media outlets and patient advocacy groups to fill the gap.

The CDC used to be more thorough in its surveillance of breakthrough cases. It tracked them all between 1 January and 30 April of this year, counting 10,262 in that time frame. But on 1 May, the agency changed its strategy to investigate only breakthrough cases that led to hospitalization or death. “This shift will help maximize the quality of the data collected on cases of greatest clinical and public health importance,” it explained.
